I’m in Naples and I’m Thinking About Replacing All My Teeth With Implants. Who Around Here Does That?

If you’re asking this question, you’re probably not looking for a dentist who can place one implant.

You’re talking about something much bigger.

Maybe your teeth are breaking down one after another. Maybe you’ve already spent years fixing individual teeth and you’re tired of doing it. Maybe you have dentures and hate them. Or maybe somebody has told you that most of your teeth are eventually going to need to come out anyway.

That’s when people start searching for things like All-on-X, full-arch dental implants, or “replacing all my teeth with implants.”

And yes, we do that here in Naples.

But I’d be careful about deciding that you need all of your teeth replaced before somebody has actually looked at your mouth.

Replacing every tooth isn't automatically the best answer

This may sound strange coming from a dentist who does full-arch implant cases, but I don't think the goal should be to see how quickly we can remove your teeth.

If good teeth can predictably be kept, that matters.

There are patients who come in convinced they need All-on-X and find out that they still have enough healthy tooth structure to rebuild their smile another way. There are other patients where trying to save everything would mean years of dentistry, repeated failures, and a lot of money spent maintaining teeth with a poor long-term outlook.

Those are very different situations.

The first thing I want to know is: What are we actually dealing with?

Which teeth are healthy?

Which ones are questionable?

How much bone is available?

What is your bite doing?

Are teeth failing because of decay, gum disease, fractures, wear, old dental work, or some combination of all of it?

And most importantly, what do you want the end result to look and feel like?

That's where the conversation should start.

What does “replacing all my teeth with implants” actually mean?

Usually people aren't talking about putting an implant underneath every single tooth.

With All-on-X, several implants are placed in the jaw and used to support a full set of fixed teeth.

So instead of a removable denture sitting on your gums, the new teeth are attached to implants.

The exact number and position of those implants depends on the patient. That's part of the planning process.

There also isn't one universal treatment called “All-on-X” that every patient gets. Bone, anatomy, bite, existing teeth, appearance, medical history and several other factors affect how I would approach the case.

That's why I don't love the idea of choosing this treatment from a website before you've even had an examination.

Sometimes the better conversation is full-mouth reconstruction

There's another group of patients who don't necessarily need all of their teeth removed.

Their teeth may just be badly worn, broken down, heavily restored or failing in different places.

Those cases can fall more into what I call Total Smile Rejuvenation or full-mouth reconstruction.

That could involve a combination of crowns, implants, bridges, veneers or other restorative treatment. The point isn't to use every procedure available. The point is to look at the mouth as one system instead of repairing one tooth at a time without thinking about where the whole thing is headed.

For somebody with significant dental problems, that distinction is important.

You're not really asking, “What do I do about this tooth?”

You're asking, “What should we do with my mouth?”

What I would want to see before recommending All-on-X

If you came into my office asking whether you should replace all your teeth with implants, I'm not going to know the answer from looking at a photograph.

I need to see your teeth, gums, bone and bite.

We also need to talk.

What dental work have you already had?

What keeps failing?

Are you in pain?

Can you chew comfortably?

Are you embarrassed by your teeth?

Do you want something fixed rather than removable?

What are you hoping your smile looks like when this is finished?

Those answers matter because two patients whose X-rays look similar may want completely different things.

After the examination and imaging, I can show you what I see and explain the options.

Sometimes that's All-on-X.

Sometimes it's rebuilding and keeping some or most of your natural teeth.

Sometimes there are a couple reasonable directions and we need to talk through the tradeoffs.

That's normal.

Ask to see cases

If you're looking for somebody in Naples to rebuild an entire smile, one of the simplest things you can do is ask to see the dentist's work.

Not stock photography.

Actual cases.

A useful case should tell you more than, “Here's a nice before and after.”

What condition was the patient in when they came in?

What did the doctor diagnose?

What treatment choices were available?

Why was one option chosen over another?

What was actually done?

And what did the result look like?

Those are the kinds of cases I think are worth studying when you're considering major dental treatment.

So, who in Naples does this?

I do this type of dentistry at Mouritsen Dental Studio in Naples, including All-on-X/full-arch implant treatment and more comprehensive full-mouth reconstruction and Total Smile Rejuvenation cases.

But I wouldn't tell somebody that All-on-X is automatically the answer just because they're frustrated with their teeth.

Come in and let us figure out what you actually have first.

If replacing the teeth with an implant-supported solution makes sense, we'll talk about it.

If there's a good reason to keep your natural teeth and rebuild them, we'll talk about that too.

For a decision this big, I'd rather give you the right plan than the fastest answer.
